Horizon EPO:

Horizon CDH HSA:
This consumer-driven product brings together a high-deductible PPO with a Health Savings Account (HSA) to create a unique health care solution which places the maximum amount of control in the hands of the policy holder. Members who elect this option make contributions to a tax-exempt HSA, and then pull from that savings account to pay for any healthcare expenses they see fit. Costs are kept low through the selection of a high-deductible plan, and then members can take that savings and contribute even more to their HSA. Plus, unused HSA funds can roll over, so your investment never leaves your hands. Horizon's HSA-Compatible PPO products include:
